Transforms for resolving references.

Class	`AnonymousHyperlinks`	Link anonymous references to targets. Given:
Class	`DanglingReferences`	Check for dangling references (incl. footnote & citation) and for unreferenced targets.
Class	`DanglingReferencesVisitor`	Undocumented
Class	`ExternalTargets`	Given:
Class	`Footnotes`	Assign numbers to autonumbered footnotes, and resolve links to footnotes, citations, and their references.
Class	`IndirectHyperlinks`	Indirect external references:
Class	`InternalTargets`	No class docstring; 0/1 class variable, 1/2 method documented
Class	`PropagateTargets`	Propagate empty internal targets to the next element.
Class	`Substitutions`	Given the following `document` as input:
Class	`TargetNotes`	Creates a footnote for each external target in the text, and corresponding footnote references after each reference.
